463 Church Entertainment Inc (hereinafter "Flash on Church Street"), O/A/: FLASH ON CHURCH STREET In consideration of Flash on Church Street issuing a membership card in my name, I, the undersigned, acknowledge and agree as follows: 1 Flash on Church Street is a private, members-only, men-only club catering to male members of Toronto's gay community 2 Entry to Flash on Church Street is restricted to members and their male guests 3 I warrant and represent that I am of the full age of 19 years and I will provide government-issued photo identification (not a provincial health card) to prove my age and my identity if and when required by Flash on Church Street 4 I will not bring any guest onto Flash on Church Street's premises unless that guest is in fact 19 years of age and by bringing that guest onto Flash on Church Street's premises I am warranting and representing that I have determined as a fact that that guest is of the full age of 19 years - Flash on Church Street may nevertheless require that such guest provide government-issued photo identification (not a provincial health card) to prove his age and his identity if and when required by Flash on Church Street 5 I shall be liable for my own misconduct and that of any guest I bring onto or into Flash on Church Street's premises and for the conduct of anyone to whom I have lent, given or purported to transfer my Flash on Church Street Card 6 The membership card ("my Flash on Church Street Card") issued...
